    • I think "never event" was chosen because it will not be reimbursed (negative compensation). But, with the law of unintended consequences, I suspect it will be used to seek tort compensation.

    • The "Never Event" is unfortunately named. In Australia a list of similar events are called Critical Indicators. I accept that they are preventable, but it is hard to say never in medicine...

  • Wow! is right. I must say, I feel attacked/defensive. I'm sorry I touched such a sensitive nerve. I think you may have missed my point entirely, which was pretty much the same as your point. Please see what I wrote about your post, which, with the exception of the straw man arguments you attributed to me, is well written. Actually, better than mine. I suppose if I had written mine better, you may not have felt it necessary to write yours :-)
